Dr. Eric Church

Dr. Eric Church

Dr. Eric Church is a board certified Anesthesiologist and has been practicing in the Phoenix valley for more than 14 years. Dr. Church received his medical degree from The George Washington University and completed his residency in Anesthesiology at The Mayo Clinic in Rochester, Minnesota. Dr. Church has extensive involvement working in the operating room as well as treating chronic pain through a wide variety of pain injection techniques. His experience ranges from, but is not limited to the spine, joints, Botox and platelet rich plasma. Dr. Church takes pride in educating his patients about their diagnosis and works with his patients on designing a reasonable and effective plan to help alleviate their symptoms.

Dr. Church brings his experience, expertise, and outstanding patient care to Pain Treatment Center of Tempe. Here, he works with patients to create individualized treatment plans to reduce their pain and improve their quality of life.

Terry S. Wilcox

Terry S. Wilcox

Nurse Practitioner

Terry S. Wilcox is a compassionate and experienced Family Nurse Practitioner specializing in pain management within a multifaceted approach. She graduated from Keuka College in the Finger Lakes in upstate New York with a Bachelor’s of Science in Nursing and has over 25 years of nursing experience. Her experience is diverse and includes hospital care, case management, insurance, home health, rheumatology back office, orthopedic care and family practice.
Terry earned her Masters of Science from Grand Canyon University with honors while working full-time and is board certified by American Academy of Nurse Practitioners. She is an experienced practitioner working in a multidisciplinary medical practice with pharmacological and non-pharmacological, movement physiology, trigger point injections as well as diet, herbal, & essential oils. Using a holistic individual approach to each patient is key to treatment.

Terry has certifications in Aromatherapy and Western Herbalism and is an avid reader and researcher. She grows some of her own herbs, loves to spend time quilting, reading about nutrition and health, working out, traveling and family time.

John Baca

John Baca

DC, DACRB, CSCS, CICE, MUAC

Dr. Baca is the Founding physician of Triad Pain Management Clinic. He has focused his practice in the treatment of musculoskeletal, acute/chronic injuries and pain of the spine and extremities. His treating philosophy is to identify the “route cause” of the patient’s pain syndrome or injury. By properly diagnosing a condition and identify a pathway to healing and restoring function; he recognized that producing a positive and successful outcome involved a multi-disciplinary approach.
This philosophy established the multi-disciplinary evidence based model of care for Triad Pain Management.

Dr. Baca was raised in New Mexico; he attended New Mexico State University on an athletic baseball scholarship and graduated in 1994 with degrees in Biology and Health Care Administration. He went on to receive a degree in Anatomy and a Doctorate of Chiropractic Medicine, from Parker University in Dallas, Texas graduating Magna Cum Laude, in 1998. He entered private practice in 1998 and has since become a Diplomate in Pain Management by the American Academy of Pain Management and has been credentialed as a Board Certified Independent Chiropractic Examiner by the American Board of Independent Medical Examiners. He has extensive experience as a Manipulation Under Anesthesia (MUA) Certified physician holding privileges at several Arizona Surgical Centers. He has performed over 2500 MUA’s and proctored and trained physicians in MUA.

Dr. Baca believes in life-long learning. He stays abreast of the latest advancements and evidenced based research to support clinical outcomes in order to bring his patients the highest standard and quality care. Due to his experience and integrity; he serves Board positions for the Arizona Work Disability Prevention Association and for a local ambulatory surgery center (ASC) that is a Medicare approved facility and accredited by the Accreditation Association for Ambulatory Health Care (AAAHC). Dr. Baca is an avid sports fan; he enjoys “road-trips”; the “great outdoors” and loves spending time with his family.

Dr. Ty Church

Dr. Ty Church

Dr. Ty Church is a board certified Anesthesiologist and has been practicing in the Phoenix valley for over 8 years. Dr. Ty Church received his medical degree from The University of Utah. He then completed an internship in general surgery at The Mayo Clinic Scottsdale, followed by residency in Anesthesiology at The University of Utah. Dr. Ty Church is experienced treating both acute and chronic pain through a wide variety of injection techniques including spine, peripheral nerve blocks, shoulder, knee, hand, ankle, hip, TMJ, platelet rich plasma, and labor pain relief.

Dr. Ty Church understands that each person experiences pain in their own way. He works to create a treatment plan specific to each patient to help them improve their quality of life and increase function in day to day living.

PTCT is proud to have both Eric and Ty Church, who are brothers and colleagues, sharing their knowledge and working together to provide the best possible care for each one of our patients.

Kevin Kerchansky

Kevin Kerchansky

DC, DACRB, CSCS, CICE, MUAC

Dr. Kevin Kerchansky focuses his career on functional rehabilitation, biomechanics and human performance. He is Board Certified by the American Chiropractic Rehabilitation Board, and is recognized as a Certified Strength and Conditioning Specialist by the National Strength and Conditioning Association.
Dr. Kerchansky is also certified by the American Board of Independent Medical Examiners, and is frequently hired as an expert to perform examinations of injured patients.
Dr. Kerchansky was born in Columbus, Ohio, and graduated from The University of Dayton in 1997 with a Bachelor’s of Science Degree in Pre-Medicine. He then attended Northwestern Health Sciences University, graduating with a Doctorate in Chiropractic Medicine. Dr. Kerchansky then went into private practice and has been a part of a multidisciplinary model of health care for nearly ten years. In 2011, Dr. Kerchansky began lecturing nationally on the functional rehabilitation of injuries. His knowledge of rehabilitation led to speaking invitations at national symposiums for a variety of professional fields including chiropractors, physical therapists, attorneys, nurse case managers and insurance claims departments.

Dr. Kerchansky is the Director of Physical Rehabilitation at Triad Pain Management Group, and oversees the multidisciplinary rehabilitation department. He enjoys studying health and nutrition, and he is very active in Olympic Weightlifting, CrossFit, and KravMaga.

Tracy Karas

Tracy Karas

PT, DPT

Tracy Karas is a Doctor of Physical Therapy (DPT) with special interest in Orthopedics. Prior to joining Triad Pain Management, Tracy owned and operated a successful private Physical Therapy Practice in Scottsdale. Because of her strong belief in a team approach to managing patient rehabilitation, she decided to join Triad as the lead PT practitioner in their multi-disciplinary Pain Group.
She recognizes that participating in a multi-disciplinary approach enhances patient outcomes and allows her to serve as an essential practitioner providing the highest quality of care for her patients.

Tracy was born and raised in Phoenix, Arizona. She graduated from Northern Arizona University in 2003 with a Doctorate Degree in Physical Therapy. Tracy is a member of the American and Arizona Physical Therapy Associations. She is also a certified clinical instructor for physical therapy and physical therapy assistant students and heads up the Student Intern Program at Triad Pain Management.

Tracy specializes in orthopedic injuries of the body. She also works in the areas of balance retraining and vestibular dysfunction for vertigo/dizziness conditions. She has a special interest in senior strength training and believes strongly in the importance of patient education and patient responsibility. Her goal is to help her patients return to previous or improved level of function while achieving pain relief through physical therapy. In her spare time, Tracy enjoys family time, especially with Claire the cat, baking, CrossFit, movies and dancing.
